Student-Athletes participating in the Countryside Cougars Football Program must complete their paperwork using the Athletic Clearance link below.

The EL2 form (Physical) must be uploaded to the Athletic Clearance link below.

Student-Athletes must also purchase the Pinellas County Student Accident Insurance from the link below.
